Can I use state and country flags for t-shirt designs, are they trademarked?

As long as it is done tastefully, you shouldn't have a problem with it. It would be smart to check the national flag laws of the flags you plan on using though. In the US, as long as it doesn't desecrate the flag, you won't have an issue.
